Med Sci Monit 2010; 16(8):BR260-270 "Long-term adaptation to high doses of morphine causes desensitization of micro-OR- and delta-OR-stimulated G-protein response in forebrain cortex but does not decrease the amount of G-protein alpha subunits"
Conclusions: Prolonged exposure of rats to high doses of morphine results in decrease of the over-all output of OR-stimulated G-protein activity in the forebrain cortex but does not decrease the amount of these regulatory proteins. These data support the view that the mechanism of the long-term adaptation to high doses of morphine is primarily based on desensitization of OR-response preferentially oriented to micro-OR and delta-OR. Wrongful Death Plaintiffs Say Defendants' Citation Supports Their Remand Motion
RICHMOND, Va. - Parents of a 17-year-old girl who died of cardiac arrest after several infusions with Johnson & Johnson's Remicade say in a June 24 appeal brief before the Fourth Circuit U.S. Court of Appeals that the case on which the defendants relied to defeat remand, although "somewhat distinguishable," nevertheless supports the plaintiffs' motion to remand (James Mack, et al. v. AmerisourceBergen Drug Corporation, et al., No. 10-1019, 4th Cir.; See June 2010, Page 4).

[The influence of inflammatory process on the ventilatory impairment in patients with stable chronic obstructive pulmonary disease]
CONCLUSIONS: Severity of hyperinflation progression may be associated with the increase of inflammatory process in patients with stable COPD. Inflammatory process may have an adverse affect on the respiratory system increasing significantly static lung hyperinflation.

[Phenotypic characterization of pyrazinamide-resistant Mycobacterium tuberculosis isolated in Poland]
CONCLUSIONS: Among M. tuberculosis strains PZA resistance was found in 6.7% of untreated patients and in 22.2% of previously treated patients. Among the PZA-resistant strains very high MIC value for PZA (>or= 900 microg/mL) was revealed for 76% M. tuberculosis strains.

Hematology: Is thalidomide combination a new option for myeloma?
Nature Reviews Clinical Oncology 7, 425 (2010). doi:10.1038/nrclinonc.2010.86
Author: Antonio Palumbo
Ongoing investigation into the use of thalidomide in the treatment of multiple myeloma has demonstrated efficacy as a combination treatment during induction and as maintenance therapy. Thalidomide–doxorubicin–dexamethasone combination therapy improved response rate; however, the development of drug resistance remains a concern. (Source: Nature Clinical Practice Oncology)

The ESTRO Breur Lecture 2009. From population to voxel-based radiotherapy: Exploiting intra-tumour and intra-organ heterogeneity for advanced treatment of non-small cell lung cancer.
Authors: Lambin P, Petit SF, Aerts HJ, van Elmpt WJ, Oberije CJ, Starmans MH, van Stiphout RG, van Dongen GA, Muylle K, Flamen P, Dekker AL, De Ruysscher D
Evidence is accumulating that radiotherapy of non-small cell lung cancer patients can be optimized by escalating the tumour dose until the normal tissue tolerances are met. To further improve the therapeutic ratio between tumour control probability and the risk of normal tissue complications, we firstly need to exploit inter patient variation. This variation arises, e.g. from differences in tumour shape and size, lung function and genetic factors. Secondly improvement is achieved by taking into account intra-tumour and intra-organ heterogeneity derived from molecular and functional imaging. Additional radiation dose must be delivere...

Special Commentary: Patient Safety and the Next Generation of HPV DNA Tests.
Authors: Kinney W, Stoler MH, Castle PE
Human papillomavirus (HPV) testing is more sensitive for the detection of cervical precancer and cancer than cervical cytology. The increased sensitivity of HPV testing and cytology combined ("cotesting") compared to cytology alone permitted professional societies to recommend 3-year screening intervals among the cotest-negative results. However, there is an increasing recognition that both clinical sensitivity and specificity of cervical cancer screening are important to patient safety and must be considered in the context of using current and future HPV DNA tests. Exquisite analytic sensitivity for HPV does not increase clinical sensitivity of an HPV test but does result in excessive test positivity and decreased clinical specificity. A recent ...

Risk of Hemolytic Transfusion Reactions Following Emergency-Release RBC Transfusion.
Authors: Goodell PP, Uhl L, Mohammed M, Powers AA
Group O RBCs are typically issued for urgent transfusions to avoid ABO-incompatible hemolytic transfusion reactions (HTRs). Identification of other clinically significant alloantibodies requires an antibody detection test, and emergency release (ER) of RBCs before its completion carries a risk of non-ABO alloantibody-mediated HTRs. We performed a retrospective review of 1,002 ER RBC transfusions involving 265 ER episodes (262 recipients) in a tertiary medical center, 2006-2008, to determine the risk of non-ABO alloantibody-mediated HTRs. A positive antibody detection test was found in 29 (10.9%) of 265 ER episodes, with clinically significant alloantibodies in 17 (6.4%) of 265 ER episodes. Fifteen antigen-incompatible RBC units were tra...
